Founded in 1985, Chapel Hill Family Medicine is a primary care practice providing comprehensive health care for patients aged four and up. We are board-certified family practice providers with a wide range of interests and skills, and decades of experience between us. We offer extended office hours to accommodate your schedule and can virtually always guarantee a same-day appointment with one of our primary care providers. We enjoy knowing and caring for our patients and their families over many years and believe that this rapport is essential to providing the highest quality of primary care through all stages of life.
Chapel Hill Family Medicine has been providing Primary Care to Monmouth County for over 35 years, including Hazlet, Holmdel, Middletown, Atlantic Highlands, Highlands, Red Bank, Shrewsbury, Keyport, Matawan, Manalapan, Rumson, Little Silver, and more!

To better care for you, Chapel Hill Family Medicine will be moving to Hazlet Family Care, which is a part of Integrated Medicine Alliance (IMA).
Our new name will be Hazlet Family Care and our new location is just a mile from our current location.

The move will be effective Monday, February 23, 2026.
